Referrals

A referral letter with your GP’s or health professional’s provider number is needed for patients to be able to claim the Medicare rebate. Patients who do not have a referral will not be able to claim the rebate.

Workers Compensation patients will need to supply their insurer information, claim number and consultation approval prior to consultation. Dr Chin is accredited with all insurance providers.

Cost of consultation will be discussed at the time the appointment is made.

Surgery fees will be discussed at the time of consultation.

Dr Chin has admission rights at Sydney Private Hospital, Strathfield Private Hospital, Sydney Southwest Private Hospital and Liverpool and Fairfield Public Hospitals.
